We aim for the referral process to be streamlined for you and your patient.
The referral can be posted, faxed, emailed or submitted through Healthlink (EDI: melville)
A dated brief referral requires:
- Referrer’s name, provider number, contact details
- Patient demographics – name, DOB, address, mobile
- The mental health concerns
- Urgent or routine
- Goal of the referral, eg:
- One-off assessment and management advice
- Ongoing shared care
- Telehealth
- Second opinion and/or Report
- Consultation liaison or case conference
Community clinic discharges
With the increasing number of discharges and rapid turnaround of public sector patients, there is a need for ongoing specialist input to guide ongoing GP care. We are happy to be involved on an annual or more regular basis to ensure medications, mental state and management are regularly reviewed.
Telehealth
With referral, we offer Telehealth consultations for all patients, and particularly our rural and remote colleagues to access timely psychiatric consultations for their patients.
Second opinions
We are happy to see surgical or medical patients, Mental Health Act patients and other second opinions as required, at various venues including ED, hospital, hospice or hostel.
Family law matters
We offer Expert Witness interview and Reports for Family Court issues, in liaison with legal representatives.
